Patients requiring a Sick Certificate for work must attend the practice in person and be seen by a Doctor who will decide whether to certify the patient. If you need a continuance of a Sick Certificate, then please contact Reception and they will place the request with your Doctor.
For a Social Welfare Certificate, we will need your PPS number and exact dates for which the Social Welfare Certificate is required. Patients need to be absent from work for more than 3 days before they receive a Social Welfare Certificate. Please contact Reception 24 hours before your Social Welfare Certificate is due. Social Welfare Certificates are submitted electronically directly to the Welfare Department.
Reports requested and paid directly by insurance companies and/or solicitors, are normally dealt with within 14 days of the information being requested. However, it may take longer depending on the size of the report, or if we are experience an influx of such report requests. We appreciate your patience in this regard. Reports requested by patients are charged according to content and complexity of reply.
